DescriptionThe Wales Cardiac Network works with partners at the interface of primary, secondary and tertiary care to provide improved care for people, of all ages, at risk or affected by heart conditions and improve outcomes to reduce premature cardiac mortality and morbidity. The work of the network is driven by a strategic leadership group, the Heart Conditions Implementation Group (HCIG) with clinical and managerial representation from health boards, commissioners, professional societies and key strategic partners from across the cardiac communities within Wales. Its agreed priorities are derived from the Heart Conditions Delivery Plan (January 2017). The main priorities include: Priority 1 - To build on the development of all Wales Clinical Pathways to delivery Value-Based Healthcare; Priority 1.1 - To develop and implement a programme of Cardiac Peer Review for Wales; Priority 2 - To implement the Out of Hospital Cardiac Arrest (OHCA) Plan for Wales; and Priority 3 - To develop and implement a Cardiac Informatics Road-map. The network works with clinical, managerial and administrative colleagues within NHS Wales to support our workstreams to meet the requirements of the Heart Conditions Delivery Plan. The project hosted on the Clinical Knowledge Manager is seeking to support the above priorities and establish a consensus for Cardiac clinical models such as Acute Coronary Syndrome.
