Description	Relevant contact for the patient or client, including formal and informal carers, next of kin, legal proxy and key workers.
Purpose	To record relevant contacts for the patient or client, including formal and informal carers, next of kin, legal proxy and key workers. These may be used within the context of a specific care plan or care pathway or may represent a more holistic vew of the key contacts involved in a person's care.
